Bipolar also medically called manic depressive Disorder and Mood disorder. In the last few years the Disorder was highly publicized and even glamorized, in particular after Stephen Fry TV Series. Undoubtedly it is one of the most fascinating scientifically but for those affected it is not really funny or glamorous!

Bipolar/Painting by Dr Taha H Malasi

I painted Bipolar when I came to study Psychiatry in the Maudsley Hospital in London 1973. My patient who suffered badly from the illness was swinging severely between depression and mania. The 3 figures represent the patient in both states of depression , in the left and manic phase in the left. When manic she was happy and elated believing she is rich, a film stars. Has diamond and pearls and offered us money and jobs. When depressed she was hopeless and helpless. The 3rd figure represent the essence of the person , the poor, the popper, in body and mind. The Dove represents her mood and my feelings . In one side the broken wing on the other the expansive extended wing like her happy  expansive mood

Mood as brain function is not just about temper and high and low, it is about the (thoughts control and Activities Control) and in reality thought are energy that control behaviors. Each one of us has his lows and high. With low mood activities mentally and physically is reduced. But mood also do strange thing, it seem to open our unconscious computer depressive files looking for all the sad and depressing ideas to see why the mood is low. Even stranger than that, if no sad or bad files found , it create them to rationalize and explain to (Himself) the bad mood!

With high or better mood we experience burst of activities and energy. Peculiarly the Brain not just  search and look for and  open all the happy (happy) Files in the brain and memories but it liberally create and generate them. Hence the sense of wellbeing and feeling happy!

In both low and high the brain believe what it has created and behave accordingly.  This is called lack of insight!!  If his thought is sad he is in dump–and he cannot help it- if he is happy he is very happy and exalted.

Of course not all depression has no reasons, psychological or social,  but like this s is not a bipolar depression? Bipolar illness is the product of genetic and chemical brain disturbances. ! It is amazing how disturbance in few chemicals, like Serotonin, nor Adrenalin and Dopamine can turn person’s life upside down.

Normally human brain characteristically produce (thought and ideas) within the limit of rationality of that particular person to produce and control rational  (activities & behaviors) with Bipolar the mood is out of control with no happy medium, that what cause most of the damage and the stress.

Affected people can present as being depressed for no apparent reason or very happy or elated and high for no apparent reason?  In both mood states, loss of control of level of energy production by the affected person brain is the main problem and it is to extremes.

When low the person is sad gloomy, lacks energy and feelings. He feels tired all the time. For no apparent reason to him his whole life -his social interaction, work, relationship completely ceased as if controlled by invisible dark force. Worse still he doesn’t know why? He get stricken by strange guilt feeling for things he didn’t’ do. In severe form of the illness he starts to believe that he is the wicked person in the world, most evil, failure. He may start seeing images of hell, fire and death, no wonder he feel hopeless, helpless and suicidal.

When the person is high , elated and ecstatic it is another story- however the loss of control is even worse. Many bipolar people t  tend to  like the high phase-as their body and mind is set free from its prison and their brain and body is full of energy, they feel they can do what they want when they want it but again the loss of control is with dire consequences .

While the depressed is paralyzed mentally and physically, the manic person is too rich with energy, thought, ideas, activities basically he  is on top of the world. Nothing beyond his reach or abilities, everything is easy done, no restriction no inhibitions moral, legal or social. Money is no trouble at all, with lack of insight everything is possible and accessible.

He may believe he is Jesus or God and get himself into trouble. The biggest problem off course they spend money they don’t have getting themselves and their families in big troubles and many do things they will seriously regret later.

Off course many artistic people, painter, poets etc  may be become more  productive, more artistic and more unusual  and original . Unfortunately again the loss of control is so severe, it deprive them from benefiting from their work and energy.

One patient disposed of his old cars on the road and bought a brand a new  car believing he won millions of pounds and a holiday around the world. He spend his family fortune and ruined everyone.

In the severest form of mania the speech will be faster than a machine gun. The can be extremely funny as their mind is quick  and humorous.  Many can be successful in less severe cases when at time has at least partial control.

The typical pattern of Bipolar is cyclic. It is fortunately described in many articles and computer self assessment programmers as (Rapid mood change) which is not true. The mood changes between mania and depression i.e. the two poles of the illness usually far from each other. Mood changes occur one at the time in the majority of cases. Attack of depression  can last between few weeks to more than a year if not treated. It can be followed by normal  mood or switch to a  Manic attack than can last months if not treated .
